It’s hard to focus on healing when the rent is late, the Medicaid renewal letter is sitting unopened, your child’s school wants another meeting, and you’re on hold with a doctor’s office for the third time this week. For a lot of families, these practical pressures are a big part of what keeps them stuck.
A case manager is someone in your corner for all of it. They get to know your family, figure out what you need most, and help you get it, so you have more room to focus on feeling better.

Case management is open to people of all ages and is separate from Pathways to Success, so you don’t need to be in Pathways to get a case manager.
Medicaid and benefits
Understanding renewal letters and deadlines, keeping coverage active, and connecting you to SNAP, WIC, cash assistance and other benefits you may qualify for.
Basic needs
Connecting you with food pantries, housing resources, utility assistance, clothing and transportation help.
School support
Coordinating with your child's school and helping you prepare for meetings like IEP and 504 plans, so your child gets the support they need in class.
Health care coordination
Help finding doctors and specialists, scheduling appointments, and making sure your providers talk to each other, with your permission.

How case management works with therapy
Case management isn’t therapy. It works alongside it. Your therapist focuses on thoughts, feelings and relationships. Your case manager focuses on the practical things that make it hard to get better.
With your permission, they stay in touch so everyone is working toward the same goals, and you don’t have to repeat your story or carry messages between people.

What does a mental health case manager do?
A case manager learns what you need, makes a plan with you, connects you to resources and services, and coordinates with your other providers so nothing falls through the cracks.
Is case management the same as therapy?
No. Case management focuses on practical needs and coordination. Many people use both at the same time.
Can you help me keep my Medicaid?
Yes. We can help you understand renewal notices, gather documents and meet deadlines. The state makes the final decision on coverage.
Can you help with my child's school or IEP?
Yes. With your permission, we can coordinate with the school and help you prepare for IEP or 504 meetings.
Do you provide housing?
We don’t provide housing directly, but we help connect you with housing programs and resources and help with the applications.
Is case management covered by Medicaid?
Illinois Medicaid covers mental health case management when it’s part of your care plan. We’ll check your coverage before you start.
How often will I meet with my case manager?
It depends on your plan. Some families meet weekly, others less often. Meetings can be in person, by phone or by video.
