Home & Community Based Therapy

Pathways to Success program is intended to serve children, youth, and young adults with medicaid between the ages of 0-21 years who have complex behavioral health needs. These youth and their families may experience disruptions in living arrangements, difficulty in school or work, struggles with social interactions, high stress levels within the home, and/or behavioral challenges. The IM+CANS serves as the decision support tool to determine eligibility.
The goal of Pathways program is to strengthen access to high quality home and community-based services through the development of a statewide System of Care focused on improving outcomes.
Pathways To Success program is administered with a collaboration between two organizations:
- Care Coordination & Support Organizations (CCSO): Specialize in Care Coordination & Support Services
- Pathways Community Provider (Blue Kite Wellness): Specialize in home & community based services namely Intensive Home Based, Therapeutic Mentoring, Family Peer Support & Respite.
Here are details of the Pathways Home & Community Services:
Intensive Home Based
A team of trained professionals come to work closely with your family at your home to help identify behaviors that create stress and disrupt family functioning.
- Helps families learn to better support the child in the home rather than through the treatment outside of the home
- The team helps the family develop and implement a plan to specifically address those behaviors.
- Helps parents and children build stronger relationships through guided interactions
- Intended to be an intensive, short – term service focusing on developing the skills necessary to keep the child safe at home.
Therapeutic Mentor
Your child and a mentor work one-on-one in the home or community to develop skills that you and your family choose.
- Mentors are positive role models in children’s lives that help them learn better social skills.
- Therapeutic Mentoring is a strengths building service that allows trained mentors to spend quality time in the community with the child, modeling positive ways of interacting with other children and adults in a variety of social situations.
- Children can work with their therapeutic mentors to gain and practice valuable skills that will help them develop positive relationships and build their confidence.
Family Peer Support
Caregivers with lived experience and/or similar family experiences provide direct support to you as a caregiver while you navigate your family’s journey to change
- Offers practical advice and emotional support from those who truly understand
- Reduces isolation and builds a supportive community around your family
- Family Peer Supporters serve as advocates, mentors, and coaches for caregivers, providing support from the peer perspective.
Respite
When your family needs a break, a trusted professional can spend time with your child at home or in the community. Sometimes a short break can help everybody get along better.
